App G Rev(07/06) Page 940-953 Page 954-957 Page 973-975 2 TA + QA : 3 _____________________ Course Grade: __________________ Initial: ______________ Math Learning Center Math 91 The Math Learning Center (MLC) can be a stimulating and rewarding alternative learning experience for students who are self-motivated, responsible, and independent. In order to successfully complete a course in the MLC, students are encouraged to attend on a regular basis (2-3 hours a day, five days a week), seek help when experiencing difficulties, and schedule their work to complete the course requirements. Textbooks: Students are required to purchase a textbook (available in the bookstore). Hours: The hours are posted both inside and outside the MLC. Log in/out at the door computer when entering/leaving the MLC. MathXL: Videos presenting the material, practice problems, tutorial clips for selected problems, and the computer quizzes are available through MathXL. MathXL is available free for use in the MLC. To use at home you must have an individual access code. If your book did not come with an access code, purchase the MathXL Student Access Kit in the bookstore or at www.pearsonstore.com. The tutorial is available for rent in CD and DVD format for those without internet access. Ask at the counter. Questions: It is your responsibility to ask questions whenever needed. Bring your book and work to the counter for a consultation with a student tutor or instructor. SID: A student identification card with picture and barcode is required when taking a quiz/test or when checking out materials. Calculators: Only a simple calculator (no +/- key) may be used when taking a quiz or test. We have calculators available for use in the MLC. Ask at the counter. Fines: A fine will be assessed when MLC materials are removed from the MLC. Cell Phone and Coats: Cell phones and are coats not allowed in the testing room. Turn off your cell phone and place it with your coat, books, and other personal items in a locker. Locks are available at the counter. Quizzes and tests: You may work on a quiz/test for as long as necessary in one sitting. The sitting is complete when you find it necessary to leave or when it is closing time. Please allow enough time! See bulletin boards in the MLC for the average time to finish a quiz or test. The first quiz must be passed prior to the last ten days of the quarter. Retaking Quizzes: You must retake a quiz if the grade is below 70%. You may retake a quiz if the grade is between 70 and 79%. To take a quiz the second time, turn in the homework for that quiz. The homework for a quiz is the multiples of 3 (3, 6, 9, 12) at the end of each section the quiz covers. You must show your work and check the answers! Answers to the odd problems are in the back of the book and answers to the even problems are available at the counter. If your score the second time is below 70%, you must take the quiz a third time (10% will be deducted from the score.). Before taking the quiz the third time, you must watch the computer video and complete the computer quiz with at least 80%. When ready to take the quiz the third time, bring the printout of the score for the computer quiz to the counter. If you do not pass the third time, meet with an instructor. Quizzes must be retaken until a score of 70% is attained. If you choose to retake a quiz, it must be retaken before the chapter test over those sections. Tests: Tests are scheduled as shown on the progress chart. Tests may be taken only once. Be aware of the time constraints and schedule tests accordingly. Students may review their quiz...
