to How fill out the GROUP STUDIES REQUEST FORM The Group Studies Request Form is often used for an instructor to teach an experimental course, a one-time offering, or a course in development. All Group Studies courses are indicated with a x94. Topics taught under a Group Studies number may only be offered a total of three times before it is necessary to convert the number to a permanent number. The following text will explain all areas of the course form. For additional assistance, contact the ASC Curriculum Office at asccurrofc@osu.edu . COLLEGE AND DEPARTMENT: Indicate the College and Department/Unit in which this course will be housed. COURSE NUMBER AND TITLE: Indicate which x94 number this course is to be taught under. The title of the course should include Group Studies as well as the proposed working title of the course. LEVEL: Level should be U (undergraduate only), G (graduate only), UG, or P (professional). CREDIT HOURS: The number of credits students will receive for successful completion of the course. QUARTER/YEAR: Check the appropriate quarter and indicate year in which this course is to be taught. If the course is to be taught more than once, please submit additional Group Studies Request Forms with the other quarters/years listed. DISTRIBUTION OF CLASS TIME: Indicate the amount of time students will spend in class. Some examples are 2 2-hr cl.; 1 3-hr. cl.; 5 cl.; 1 2-hr cl., 1 2-hr lab. PREREQUISITES: Any course that must be successfully completed before a student may enroll in this course. When in sequence, they should be listed in the following order: Home Academic Unit, Other Academic Units, General and Requirements. If there aren t any prerequisites, this line should be left blank. EXCLUSION CLAUSE: Types of students not eligible to enroll in the course. Examples may include the other unit if the course is cross-listed, or students with specific criteria. If any student is allowed to enroll, this line should be left blank. LIMITING CLAUSE: Types of students eligible to enroll in the course, An example is: open only to Psychology majors. If there is no limiting clause, this line should be left blank. REPEATABLE: The total number of credit hours a student may repeat a course for credit. If the course is not to be repeatable, this line should be left blank. GENERAL INFORMATION STATEMENT: Any other information that should be communicated to students about the course, including GEC category if necessary. Examples are: Taught in English; Does not count toward major. Discuss the course with any units that may have related or jurisdictional interests and attach any letters of concurrence. RATIONALE: Indicate the rational for proposing this topic. SCHEDULING INFORMATION: Fill out all sections of Scheduling Information with your Scheduling Coordinator in your unit/department. APPROVAL PROCESS: Obtain appropriate signatures from the Undergraduate and/or Graduate Studies Chair and the Unit Chair/Director. Once the form is complete, fax this form to the Curricular Assistant or Associate Dean in your College and email your syllabus, rationale, and any other supporting documentation to him or her. He or she will approve the course and forward the documentation to the ASC Curriculum Office at asccurrofc@osu.edu.
