2The main difference between systemic and individual problems is that systems problems usually affect more than one individual, even though the cause of the problem may be from a single individual, whereas individual problems are those that affect only one individual (Vincenzes et al., 2019). In this case, a systems problem would lead to medication errors across the board whereas individual problems would only affect the particular nurse. To decide whether this is a system problem or one that is simply related to the individual nurse, the first step would be to collect any data on medication errors in the unit.
