2 The main difference between systemic and individual problems is that systems problems usually affect more than one individual, even though the cause of the problem may be from a single individual, whereas individual problems are those that affect only one individual (Vincenzes et al., 2019). In this case, a systems problem would lead to medication errors across the board whereas individual problems would only affect the particular nurse. To decide whether this is a system problem or one that is simply related to the individual nurse, the first step would be to collect any data on medication errors in the unit. Data is integral towards understanding the underlying factors in a problem (Brenna & Bakken, 2015) This data would be crucial in helping the nurse understand whether there have been any errors and if so, who caused the errors.
