Central Venous Catheter Blood Draw Checklist Step S U N P Comments Gather equipment. Alcohol pads 10ml syringe or size appropriate for lab studies Prefilled and labeled 10cc normal saline syringes Vacutainer blood transfer device Vacutainer blood collection tubes Lab Requisition with specimen labels Check the name on the patient’s identification band. Check the identification number on the patient’s identification band. Check the birth date on the patient’s identification band. Ask the patient to state his or her name and birth date, based on facility policy. Perform hand hygiene. Put on gloves. Choose Port. Assess all medication and infusions before selecting a port. If fluid is infusing through a single lumen catheter, stop the infusion, clamp the line & remove it until drawing has been completed. If there is an infusion into a multilumen line, stop the infusion & clamp the line until the draw is complete from an unused lumen. Have patient turn head away from CVC site during procedure. Clean port with alcohol swab for 15 seconds.
