Medication errors are considered one of the most challenging yet highest priority issues that need to be addressed in the different setting of healthcare that involves medications. The U.S. Food and Drug Administration (FDA) has estimated that medication errors cause at least one death every day and injure approximately 1.3 million people annually in the U. S[CITATIONNic \p 166 \l 1033 ]. With the staggering number of medications prescribed by doctors, dentists, and nurse practitioners it is a wonder that it is not higher. However, every effort must be made to prevent medication errors that can harm and even kill the people who are taking them. In investigating medication errors, the Joint Commission has identified several reasons why medication errors are so prevalent among patients.
