CASE III INCIDENT REPORT 2 Case III Incident Report Incident reports are a necessary function of any successful safety program they help you make sure that you cover every aspect of the incident and determine the root causes which contributed to the incident occurring. According to the National Safety Council (2009), “An important part of any incident investigation is the causal factors analysis or determining those causes that directly contributed to the incident” (p.155). In the tool truck incident there was a combination of factors which contributed to the incident. Equipment (NSC, 2009, p.156-157) 1) Was a hazardous condition[s] a contributing factor? In this case the answer is yes because there were some issues with the tool truck which may have prevented the accident. 2) Did the design of the equipment create operator stress or encourage operator error? Yes the truck design did encourage operator error because the seat was big enough to allow two people to sit in it but if the driver moved over he would be off-set to the steering wheel (NSC, 2009, p.561). Corrective action in this case could be to place a padded bar to the right of the where the driver sits or put in a smaller seat which could only accommodate one person. 3) Did the general design of the equipment contribute to a hazardous condition? Yes, the general design of the truck did contribute to the accident in several ways. First, the truck is equipped with a seat large enough to hold two personnel. Second, the truck did not
