Keeping in mind that in this case the incident event is different than the injury event (National Safety Council, 2009, p. 154), please give consideration, as a non incident related corrective action, the following. In the case of the equipment it should be noted that the lack of a personal restraint device contributed to the significance of injury. While this alone would not have decreased the likelihood of the incident occurring it certainly could reduce the severity of any future occurrences. As a result this equipment should be reviewed for applicable safety features and contact the vendor to verify recent or available upgrades. Pre-use safety inspections should be implemented as a
