During the aftermath of this accident investigation key information was collected and will be addressed. As we look into the casuals factors that may have led to and/or prevent this or future accidents of this types. The first area to look at will be the equipment used during the time of the accident. Did the truck in use during the accident have any defective equipment at the time of motor vehicle accident (MVA)? The after action inspection did not find any defective equipment at the time of the MVA. Did the vehicle have the needed or required safety equipment for operation? It was noted that at the time of the MVA, the truck was not equipped with seatbelt. This resulted in the driver being throw for the vehicle after impact. Is there a prior to use inspection of the vehicle to indentify hazards? There is no indication that a prior to use inspection is being performed on the vehicle or that there was no seat belts were addressed prior to the accident (National Safety Council 2009). The environment that the vehicle was being used in during the time of the accident played a large role in the accident. The construction that was underway played a role. The fact
