INCIDENT INVESTIGATION 2 INCIDENT INVESTIGATION CASE STUDY III employee to conduct and do their daily tasks in a safe manner. Unfortunately accidents and incidences can still occur due to many reasons such as rushing, distractions or just laziness. After every accident or even near-miss, a fill Incident Investigation should take place to find the root cause and hopefully eliminate the same situation from happening again. This case study involves an incident that encompasses many errors in judgement and safety precautions. Case Study III involves many factors that ended in the unfortunate fatality of a tool truck driver. Immediately after the accident, an emergency ambulance was called to take the driver to the hospital, the passenger was also checked out for injuries. The passenger was fine and taken to a quiet office area for questioning. He was obviously under a lot of stress and we gave him some
