Unformatted text preview: evaluation form is included in the internship guidelines package. All of these requirements, the journal, internship report, and your host company evaluation are due to me by close of business on Monday, August 6th. Plan ahead for accomplishing these requirements! I need these items on August 6th so I can meet the universities deadline for submitting grades. If you miss the submission date, you will receive a grade of F for the course. If you mail it to me, plan accordingly by mailing it early enough to meet the deadline of August 6th. You can mail it, email it, fax it, or drop it off at the Management Sciences Receptionists office (Cara Switzers office). Whatever works best for you as long as you meet the deadline. Let me know if you have any questions about anything. Please review the guidelines and if you still have questions, please contact me. Good Luck. Malcolm...
