FOCUS/NursingDiagnosisExpectedOutcome/s (ofPRIORITYdiagnosis)PLAN: NursingInterventionsRationale1. Decreasedcardiac outputrelated to impairedventricular filling, contractility, oroutflow obstructionas evidenced bytachycardia, dysrhythmias, andhypotension.2. . Impaired gasexchange alveolaredema secondary toincreased ventricularpressure asevidenced bydyspnea, orthopnea, and DOB even atrest.3. Ineffective tissueperfusion related topoor cardiac outputas evidenced bycyanosis on oralmucosa and nailbeds, generalizedweakness, and pittingedema on abdomenand both extremities.Priority: According toABC, maintainingcirculation isimportant in movingoxygen to the tissuesand carbon dioxideout of the body. Asidefrom that, addressingthis problem mayaffect/address as wellthe other identifiedproblems.LTO:After 3 days ofnursing interventions, thepatient will demonstrateadequate cardiac outputas evidenced by BP of atleast 110/80, PR of60-100, dysrhythmiascontrolled, urinary outputof increase to at least300-500 ml.
