Assessment instructionParticipant NameResources required to undertake this assessmentParticipants must have access to these resources:Copies of the activities, questions, projects nominated by the trainer/assessorRelevant organizational policies, protocols and procedural documentsAn appropriate workplace or simulated workplaceAssessment instructionsParticipants should respond to the formative and summative assessments eitherverbally or in writing, as agreed with the trainer/assessor. Written responses can berecorded in the spaces provided (if more space is required attach additional pages) orsubmitted in a word-processed document.Participants should also undertake observable tasks that provide evidence ofperformance.
