Alhamdulillah. Thank you to Allah SWT, whom with His willing giving us the opportunity
to complete our group assignment. This assignment has a lot of efforts have been done from
various parties involved in contributing information for us. Thankfully, we are able to finish this
assignment on the time given.
Firstly, we would like to express our deepest thanks to Dr. Zaimy Johana, lecturer’s
subject of Occupational Safety, Health and Environmental Management (OPM538) who had
guided us in carried out this subject.
Our sincere thanks also goes to Mrs. Izzaty for her kindness which also helped us a lot
in finishing this group assignment and gave us valuable information. Without her cooperation,
we could not complete this task easily.
Besides, deepest appreciate to all of the group members which is Hilmi Hakimi, Vanessa
Shellen, Azra Hannani, Izzatul Asani and Cornelia Lipang for cooperation, believing and full of
information required in the completion of this report from the beginning till the end.
Thank you also goes to our classmate and family that has contributed by supporting our
work and helping us throughout the assignment progress till it is fully completed.

Mrs Nurul Izzaty Binti Zaaba, a safety and health officer with over nine years of working
experience in the manufacturing industry. Our group has chosen Mrs. Izzaty as the competent
person for this assignment because she is the DOSH officer in Lee Kum Kee SDN. BHD. and
has been certified by DOSH Malaysia. During the interview, we have discussed about her being
a competent person, educational background, the challenges and also experience as DOSH
officer and many more.
Overall, we have gained from the interview that Mrs. Izzaty taught us a lot about how the
manufacturing industry operates, as well as the importance of employee safety and health. Mrs.
Izzaty begins the interview by explaining what OSHA is and what high risk, medium risk, and
low risk are being used as to evaluates all of the circumstances that could happen in the
workplace. Mrs. Izzaty also mentioned one of the memorable accidents that happened while
she was working in the industry that she will never forget. The incident really makes her more
responsible now when it comes to the safety of the workers and also the workplace.
