NR 603 Week 6 Patient Information: K.G., 18 year old, Female, Caucasian, Amerigroup Medicaid Subjective Chief Complaint:“I just don’t have enough energy to show up to volleyball practice.” HPI: K.G. is a 18 year old female who is a Senior in High School and lives at home with her mother, father, and older sister who is currently away at college. Her mother scheduled her appointment and accompanies her today. K.G.’s mother is concerned about K.G.’s ongoing fatigue, loss of interest, increased sleeping, and decreased appetite with weight loss. K.G. states she just hasn’t wanted to leave her room the past 2-3 weeks. She states she’s felt like this before, approximately 3-4 times per year but for some reason this “funk” has lasted longer. Her mother reports they have a history of mental illness in the family and she is very concerned with how K.G has been acting recently since she usually is high and full of energy, a little on the wild side, our risk taker.
