Wound Wet DressingSupplies:-Tape- Saline-Bio Bag- Q Tips-Measuring tool- Tweezers-Gauze- Biohazard bag1.Intro + Check Allergies2.Assess for Pain. (Give medication3.Chuck pad under patient4.Emesis basin at end of wound5.Tape Q Tips to edge of side table.6.Apply gloves7.Pull dressing off, towards patient (Check for COCA)8.Use tweezers to remove gauze inside wound (count how many removed and assess for REEDA)9.MEASURE IN CM – Length, Width, & Depth.10.Check for tunneling11.Prepare to irrigate- Normal saline check exp, palm label, lip it to trash can THEN pour intocontainer12.Irrigate wound with normal saline in ZIGZAG until clear13.Pat dry with Gauze around wound14.Open sterile box15.GO STERILE Don gloves16.Place drape on side table (outer side first)17.Remove items from sterile box18.Once box is empty, leave one gauze in box and pour normal saline into box with gauze.19.Palm Normal saline and pour into box.20.Take tweezers to hold gauze and cotton swab to help place gauze into wound21.Apply sterile dressing, blue line up22.Tape down (horizontally)23.Initials, time, and date on tape.24.

Place drape on table 11. Don Sterile Gloves 12. Open swabs (6) 13. Clean wound (3 swabs) 14. Use tweezer and clean pin rose (3 swabs) 15. Place gauze on wound 16. Place new bandage on wound (Blue to the sky) 17. Place tape 1 inch apart. 18. Clean up19. Call bell new Pt and bed lowered. Document: - COCA/REEDA- How pt tolerated....
