Chapter 9: Schizophrenia and Related Disorders Case Report: David Marshall 22-year-old who was brought into the emergency room by his parents who were troubled by his odd thinking and behavior. He was demanding to see “Zoroaster” and was annoyed when Dr. Tobin entered. His arm was moving in sweeping circular motions in front of him, seemingly beyond his control. He kept talking about his mission to save human kind from “thools” from the planet Dortanus. David was in a psychotic state and therefore unable to give accurate details of his emotional state or an accurate description of events in his life. Dr. Tobin asked to speak to his parents alone and David stormed out. His parents said he often left home but returned within a couple of days. He had flunked out of the first semester of college because he spent most of his time in his room. When he was finally found, his parents called an ambulance to bring him back to the psychiatric hospital. As a child David was neat and clean and bathed every night, but by adolescence he would go weeks without bathing. The only family history of mental illness was his mother’s sister who was hospitalized for hearing voices and having strange beliefs. During Dr. Tobin’s attempt at a psychological assessment, he would not give any information about himself other than that relating to his delusions. Dr. Tobin diagnosed David as having schizophrenia, based mostly on the course and symptoms of his disorder. Treatment: David was prescribed medication, which surprisingly he took voluntarily. He was kept in the hospital for 3 months. As he took the medication his thoughts became more lucid and he was able to carry on a conversation without thoughts of Zoroaster. When David’s symptoms were more under control, he was discharged under the condition that he attend a day hospital program for 6 months in addition to seeing Dr. Tobin for weekly psychotherapy sessions.
