*Activity 1* Due on April 7 th , 2008 Your risk for disease Please collect a 3-generation family history for the major diseases in your family (The information is important, not the diagram of a family tree, there is an example for you below). Based on this information and your own behavior, pick the disease which has the most relevance to you and complete the questionnaire on the website listed below. If some of you cannot reach your family for some reason, please let me know beforehand. A 3-generation family history includes the following: * First-degree relatives (e.g., children, brothers and sisters, and parents). * Second-degree relatives (e.g., grandparents, aunts and uncles, nieces and nephews, and grandchildren). * Third-degree relatives (e.g., cousins, great aunts, and great uncles). For any relative with cancer, collect the following information: 1) Type of each primary cancer. 2) Age at diagnosis for each primary cancer. 3) Where the relative was diagnosed and/or treated.
