2 Community assessment based Windshield Survey Community health assessment ( CHA), also known as community health needs assessment (CHNA), include local state, tribal, or regional health assessment that use systematic, comprehensive data collection and analysis to identifies key health needs and issues.(CDC). As per CDC, CHA uses the following principle to determine the issues: It involves different hospital, medical entities, and social worker to support all phases of community health improvement. It includes assessment, planning, investment, implementation, and evaluation It involves larger population interventions and measurable results and includes a targeted focus to address disparities among subpopulations Use of evidence-based interventions to improve the outcome and encouragement of innovative practices that can bring the good overall result. Data is pooled from diverse public and private sources to evaluate and assess continuous improvement process Windshield Survey component for the Community health assessment: This method is based on a scientific module.This process involves utilized to gather data within communities. This assessment demand some travelling throughout the targeted community and collecting data so that one can vision, understand and sense the surrounding and their needs (Gibson & Thatcher p410) . The Quad Cities is a region formed from four counties in northwest Illinois and Southeastern Iowa.It consists of four cities: Davenport and Bettendorf in Iowa, and Rock Island, Moline in Illinois.
