Trillium Health Partners is a prominent hospital with a proven record of excellence in performance,

age, as more and more individuals live with chronic conditions, we are anticipating the unavoidable
impact of change.
This strategy plan charts a trajectory for Trillium Health Partners (THP) over the next 10 years and
outlines the goals for reaching the milestone. The relationship between the organizational plans of
the national health policy, strategy, or plan (NHPSP) and the operational objectives is operational
planning. It is everything about converting the strategic-level strategy to chores to be completed. The
