2015 Application Page 1 Revised June 2014 FIN-125 APPLICATION WORKBOOK PLEASE INCLUDE THE APPLICATION CHECKLIST SPREADSHEET AS THE TABLE OF CONTENTS IN THE FRONT OF EACH BINDER. ALL REQUIREMENTS CHECKLIST This spreadsheet and each of the other spreadsheets discussed below and included in this workbook offer guidance in putting together your application correctly. Please read these instructions completely. An application consists of one (1) tabbed three-ring binder. The one binder will have originals of each paper copy exhibit that is required. In addition, most exhibits are required to be saved onto CD-R(s) or flash drive (s) and included in the binder containing the original paper copy exhibits. Please note CD-R(s) and flash drive(s) will not be returned to the applicant. The All Requirements Checklist spreadsheet details whether or not an exhibit is required. If an exhibit is not always required, it details when an exhibit may be required. It details the format in which the file should be saved. Please note that some exhibits may be required to be saved in more than one format (Ex. One file in Excel and one file in Word) and that for some exhibits you may have an option to save the exhibit in one of several formats (Ex. One file in either Excel or Word). Also, if you have multiple exhibits for the same category, please place a sequential number at the end of each of those file names. For example, if you had multiple sites, you might have multiple site plans. You would name the first site plan file "5c Site Plan 1" and the second site plan file "5c Site Plan 2."
