Case Management Overview PaperIntroductionThe purpose of this paper will be to give a brief overview of case management. This paper will first discuss the various roles and responsibilities that a case manager may have or should have. Then, it will explain the importance of being able it identify problems and causes of behavior theoretical models. Finally, this paper will define what a special population is and what potential issues a case manager should be prepared to address or attend to when needed.Roles and ResponsibilitiesThere are many different roles and responsibilities for a case manager to partake in, these are thirteen of them which are briefly covered below.1. Outreach worker: Reaches out in the field to find people who need help and refers them to the services they need, then they follow up with those individuals to make sure they are receiving or received the appropriate services that they required; if not so they can refer them elsewhere, where the services will be more suitable for the individual’s need.2. Broker: Helps people get connected to the services they need; also helps the agencies or service providers to communicate with the clients. The broker can make contact with the agencies or service providers necessary that offer the services for the clients.3. Advocate: Fights, pleads for change, services, policies, rules, regulations, and laws that will benefit the client.
