PDSA Cycle Template
Directions: Use this Plan-Do-Study-Act (PDSA) tool to develop the initial plan for making an evidence-based
quality improvement. This tool should be completed by the student with review and input by the course faculty.
Complete the steps identified in the PDSA table below. Document your responses in the corresponding column.
Then develop a proposed plan to meet your aim. Remember that a quality improvement project will usually
involve multiple PDSA cycles. This FIRST PDSA cycle is meant to help you develop the QI Capstone Plan.
Subsequent PDSA cycles can be used to test the proposed improvement/change using plan, do, study, and
act approach AFTER the Capstone course is completed and approval from both unit leadership and
organizational IRB are obtained.
