A medical record is a systematic documentation of a patient’s medical history and care. Contents of these records include: demographics, medical history, medical encounters, orders, progress notes, test results, and other information important to the patient’s care. 2) List professional credentials available to health information personnel and distinguish among them by outlining the sponsoring agency, educational criteria, and type of jobs performed. Health Information Administrator (RHIA) Bachelor’s degree from accredited program Typically in management level positions requiring interaction with colleagues on the medical, financial and administrative staffs More than half of the current RHIAs are directors, managers, or consultants, with nearly 31 percent serving as health information management directors. Health Information Technician (RHIT)
