Paper: Date and course number on title page Headers In-text citations APA style Title should be name and diagnosis Follow outline Just follow and fill in Family Hx; Very succinct, not Hx of CL in family Social Hx; Succint, patient’s history (highlights of education, legal, etc.) Medical Hx; CL’s, not family Past psychiatric Hx; CL’s… inpatient/outpatient, etc. Hx present illness; a little bit of elaboration of chief complaint (why now?), what symptoms? Etc? M.S.E. (The mini-mental status exam is just ONE part) Instrument, Measurement, Rating scale… make sure it’s mentioned (what is it, what’s it measure, etc.?) Why does this diagnosis apply and not another one apply? Follow DSM-IV for order of multiple diagnoses Full 5 Axis and ICD9 and ICD10 (for psychiatric or medical or just psych)
