CC Chief complaint- I feel like I can't get enough air when I breathe. 2 O Onset of symptoms- Going on for the last 3 months, but lately it seems to be getting worse. 3 P Palliative? Sitting up and not doing anything is the only way I can get comfortable; sleep on 4 pillows 4 P Provocative Even the slightest exertion makes me struggle to breathe; worse if I end up flat on back during the night 5 Q Quality- feels like I’m not getting any air— like I’m being smothered . 6 R Region- in my chest— like air can’t get out into my lungs. 7 S Severity- during the day my breathing is bad, but at night, in bed, it is really terrible - I wake up struggling to breathe 8 T Temporal/progression - it has been much worse for the past week , and really bad last night and today . 9 A Associated symptoms . legs are very swollen; I feel weak and tired; appetite is terrible; eating little but craving salty foods; just don’t feel well A Attribution-What does patient think?
