Antibiotic Case Study 1 The first part of the nursing process is assessment; therefore, as in the case of the scenario above, several questions would need to be asked of the patient. There is a mnemonic device that is helpful in guiding the types of questions that would need to be asked; this device is called OLDCARTS. OLDCARTS stands for onset, location, duration, characteristics, aggravating/associated factors, relief, temporal, and severity. From this helpful assessment guide, the questions would then be when the pain started, how often the pain occurs, nature of the pain, if it radiates to other areas of the body. Are there any activities that make the pain worse or does anything diminish the pain? Are you taking any medications for the pain? The location and severity of the pain are already known, the location being in lower abdomen and the severity being ranked at a 3/10. Then ask about social and history: history of alcohol, history of drug use, sexual history.
