BA 4101 – COFFEE WARS GROUP MEMO EVALUATION FORM Below are the criteria by which your Group Memo will be evaluated. If you have been responsive to a criterion, place a check mark (“ ”) at the underscore on the left. If you have not, place an “X” in that same space. Do the same with the Group Memo CHECKLIST (printed on reverse side). Your group number should NOT appear anywhere on these forms or the memo except on the back (outside) of the entire memo in the lower right hand corner (see check list for further format instructions). Staple this one page form (with the checklist printed on the reverse side) to the FRONT of your completed memo. If all is in order, having followed ALL the stated instructions, you get +5 points.
