human actions” (IAEA 1988, p.19). On reflection of the causes of the Three Mile Island (TMI) accident, Kemeny et al. (1979) concluded that many incidents and accidents in NPP applications were due to insufficient recognition of the importance of the human beings who managed and operated the plants. Although issues related to human-system interfaces (HSI) in NPP environment have drawn more and more attention and many measures have been taken to prevent human errors since the TMI accident, incidents caused by human actions continue to occur. In an attempt to understand the nature of HSI related human errors in NPP control rooms and then propose effective corrective actions to prevent the errors and/or mitigate their consequences, the present study took a statistical approach to the errors to identify their categories, occurrence patterns and trends, and reveal potential hidden interrelationships between them and their causal factors.
