Mr. R., age 52, is undergoing a routine physical examination for his employer. During palpation of the prostate, the doctor noted a hard nodule on the gland’s periphery. Lab tests revealed an elevated PSA level. The remainder of the examination did not reveal any other abnormalities. Discuss the presentation of Mr. R’s cancer of the prostate. The finding of a hard nodule on the prostate gland’s periphery during digital rectal exam is the most common presentation of prostate cancer. Prostate cancer rarely obstructs urine flow due to it’s location, especially in the beginning stages. Mr. R also has an elevated PSA level which indicates a prostate tumor (benign or malignant). Discuss the predisposing factors for prostate cancer and identify the high risk groups. The predisposing factors are - age (over 50), genetics (inherited mutation of the HPC1 gene), high androgen levels, recurrent prostatitis, increased insulinlike growth factor. It’s also more common in the African American population.
