The rise and decline of America’s once elaborate federal, state, and local partnerships in the delivery of public health services are described, as well as the efforts of private and voluntary agencies. Also discussed are the barriers to effective preventive services that result from the lack of population perspective in the U.S. healthcare system. The chapter concludes with a discussion of public health challenges and goals, emerging issues, and the changing role of government.
