Question 4 Select an evaluation of counseling services instrument from the text. Describe the purpose, content, and how you might use it in your role of PPS Professional. In order to improve and enhance the strengths of the counseling and guidance program, thousands of instrumentations are introduced and established to collect data and assist counselors to evaluate the services provided. In the following, I would like to present one of the instrumentations – the Life Career Plan Conferences With Students and Parents (Gysbers, & Henderson 1992). For the Life Career Plan Conferences With Students and Parents, it is basically separated into 2 sample surveys, one for student and one for parents, including 1) Individual Planning Conference Evaluation: Student Form and 2) Individual Planning Conference Evaluation: Parent Form.

With the results analyzed, further planning conferences can be adjusted and modified in order to meet the needs of both the students and the parents. Based on the students and parents evaluation forms, both contain 5 short descriptive sentences: for example, the high school requirements, the understanding of SAT / ACT scores, the post-high-school options, the consideration of future goals and the selection of courses.
