GEORGE MASON UNIVERSITY School of Nursing NURS 343 Pharmacology Female and Male Reproductive System Drugs Female Reproductive Drugs (Ch 69 & 70) Estrogens & Progestins Mechanism of Action/Effects/Use Bind to estrogen receptors causing wide-ranging effects including development of female sex characteristics, effects on bone (accelerated closure of epiphyses, decreased bone resorption etc.) Used in combination for oral contraception, prevention of severe dysmenorrhea; previously routinely used for hormone-replacement therapy (ERT/HRT) to ease symptoms of menopause, now only recommended for short-term symptomatic relief of menopausal symptoms and for shortest length of time possible; palliative treatment of advanced prostatic and breast cancer Up to 99 – 100% effective when taken consistently as oral contraceptive (“mini-pills” containing progestins-only have higher incidence of pregnancy and side effects) Progestins alone may be used to treat dysfunctional uterine bleeding (many causes), amenorrhea, palliative treatment of some cancers (e.g., uterine, renal) and endometriosis Side/Adverse Effects SE: Nausea, bleeding, fluid retention AE: thromboembolic events, breast/uterine cancer Contraindications History of thromboembolic conditions (e.g., thrombophlebitis, stroke), coronary artery disease, liver disease, breast or uterine cancer, pregnancy (category X drug) Nursing Management ethinyl estradiol with norethindrone (Ortho-Novum) CLASS estrogen-progestin THERAPEUTIC USE prevent conception MECHANISM OF ACTION inhibits ovulation AE nausea, bleeding, weight gain, breast tenderness medroxyprogesterone acetate (Depo-Provera) Prevent conception 1
