PREVENTING MEDICATION ERRORS Minimize verbal/telephone orders, repeat order & spell out name List indication for use next to each drug order Avoid medical shorthand Never assume anything in drug order Never try to decipher illegibly written orders When in doubt always double check with prescriber, pharmacist or the literature

PREVENTING MEDICATION ERRORS (cont.) Use 5 Rights Always read the label 3 times No trailing zeros but always use leading zeros for decimal dosages Encourage both trade & generic names with orders Listen to the client Be alert Check allergies & update chart as needed
