Unformatted text preview: DSM-IV Symptom Checklist Instructions Introduction You are rating on the presence of specific criteria, and symptoms within each table based on the chart. How to Use the Ratings of 0, 1, and 2 0=Absent or False Leave the box blank (a rating of 0 is implied) if the symptom/criteria is not present. Rate a 0 when it is useful to do so, e.g., for a criterion within a table in which a diagnosis is in question. 1=Subthreshold Rate a 1 when there is only some evidence of the symptom/criteria but not enough to warrant a rating of 2. 2=Threshold or True Note: Rate a 2 when there is ample evidence in the chart that the symptom/criteria are clearly met.
