American youths, around 18,200,were diagnosed with type I diabetes and around5,800 have been diagnosed with type II diabetes between 2014 and 2015, according to reports(ADA, 2022). The root cause of Diabetes is when thebody is not producing enough insulin,which is a severe long term condition. The body cannot convert blood sugar into energy and thisresults in high blood sugar levels. The three main groups arediabetes type I, diabetes type II,and gestational diabetesAn autoimmune disorder,type I diabetes typically manifests in infancy and childhood.This type cannot be avoided and is thought to be brought on by an autoimmune reaction. Type Idiabetes patients frequently experience rapid onset of symptoms and require daily insulininjections to survive (CDC, 2022).Type II diabetes typically occurs in adults and develops over several years. However, therate at which type II diabetes is diagnosed in children, adolescents, and adults is gradually rising.Most of the time, this kind of chronic condition is caused by one's lifestyle. It can be avoided bymaking healthy changes like weight loss, healthy diet, and physical activity (CDC, 2022).Pregnant women without diabetes prior to birth can develop gestational diabetes. Oncethe birth has occurred gestational diabetes typically disappears. Notwithstanding, it expands thelady's gamble and the child's gamble for creating type II diabetes in later years (CDC, 2022).Depending on the person's blood sugar level, diabetes can cause a variety of symptoms. Theremay be people with type II diabetes who don't show any symptoms. On the other hand, peoplewho have type I diabetes typically experience more severe and rapid onset of symptoms.Increased dehydration, weight loss, urination, mood swings, and other symptoms are all signs oftype I and type II diabetes. Long-term complications of diabetes include nerve damage,heart
3disease, cardiovascular disease, kidney damage, and others(Mayo Clinic, 2022).The telehealth system that will be implemented into the system is an app and computerprogram that collects the information of the patients with diabetes in Philadelphia. Therelationship between a patient's diabetes metrics and their food sources will be the primary focus.A health program which focuses on patient social determinants of health and diabetes metricswill be proposed by the team to be implemented in partnership with PCP offices in thePhiladelphia, Pennsylvania, region. Which then will be transformed into a phone and computerapplication. The study's conclusions will be based on both qualitative and quantitative metrics.Engagement:Because of the characteristics of the disease, there is a strong connection between foodand diabetes. As a result, these organizations that treat diabetes need to know that food accesscan affect the patients in these communities, in which they serve. The United States Departmentof Agriculture states that food insecurity is the inability to obtain sufficient food for an active andhealthy life. In light of its relationship with unfavorable social and wellbeing results, food frailtyis a serious general wellbeing worry that gets from joblessness, neediness, and pay shocks, whichcan forestall satisfactory admittance to food, in any case, the overall commitment it needs to
