Acute Kidney Injury -Rapid decline in renal function resulting to the collection and accumulation of metabolic waste in the body Manifestations: Prerenal Azotemia Intrarenal (Intrinsic) Postrenal Azotemia Hypotension Tachycardia Decreased CO Decreased CVP Decreased UO Lethargy Sodium decrease 10-20 Gravity: >1.030 Decreased UO/ absence Edema Hypertension Tachycardia SOB Distended neck veins Increased CVP Weight gain Crackles Anorexia N/V Lethargy/ LOC change Sodium <40 Urine gravity: <1.010 Oliguria/ intermittent anuria Assess uremia (S&S) Lethargy Change in urine stream Presence of blood/ particles Urine gravity: 1.000-1.010 Intra/Postrenal- increased urine gravity, ECG change: tall T wave, pulmonary edema, friction rub, flank pain, increased potassium. Laboratory: Creatinine M: 0.6-1.2 mg/dL F: 0.5-1.1 mg/dL Gradual increase of 1-2 mg/dL every 24-48 hrs. May increase 1-6 mg/dL in 1 week or less. BUN 10-20 mg/dL Often increases by 10-20 mg/dL at the same pace as serum creatinine. May reach 80-100 mg within 1 week. Sodium 135-145 meq/L Normal, increased, or decreased Potassium 3.5-5.0 meq/L Increased Phosphate 30.-4.5 mg/dL Increased Calcium 9.0-10.5 mg/dl Decreased Magnesium 1.3-2.1 meq/L Increased
