NHM 201 Name: Monica Bishop Preconception & Pregnancy Case Study 40 points Read the case studies and answer the questions. Pt.: Susan is a 38-year old female, Caucasian, married mother of one. Based on her primary care physician’s referral, Susan makes an appointment with a registered dietitian/nutritionist for help in developing a meal plan appropriate for promoting weight loss to increase her chances of becoming pregnant. Susan and her husband have been trying to get pregnant for the past 6 months. She currently experiences irregular menstrual cycles. Susan is a vegetarian who consumes fish and milk products but does not consume any other animal products. She works 40 hours per week at a retail clothing store. She lives 45 minutes away from this store. Her husband works late most nights. Pt. reports “no time to exercise” and reports she believes “I am a sugar addict.” Pt. reports numerous attempts at weight loss, including meal replacement products (Slim-Fast) and group weight loss classes (Weight Watchers). Pt. reports weight loss of no more than 10 lbs. with each attempt. Obstetrical History: Gravida 2, Para 1. In her second pregnancy, she had a spontaneous abortion at 12 weeks, which happened 1 year ago. Her living child is age 4 years old. She gained 60 pounds with her first (and only successful) pregnancy. Family History: Pt. lives with husband and daughter. Physical Exam: Height: 5 feet, 4 inches; current weight: 185 lbs. BP: 140/90. Waist circumference: 39 inches.
