Pelvis 4 Field Supine Diagnosis and Stage This patient is diagnosed with rectal cancer. He has undergone a colonoscopy that was followed with surgery for a lower anterior resection of the rectosigmoid in August 2016. After his colonoscopy he had a PET and MRI done. His rectal cancer stage is IVA, T3, N1a, M1a. Medical History His medical history includes rectal cancer, fissures, chemotherapy, diabetes, hypertension, thyroid nodules and ulcers. His father and mother are alive and his paternal grandfather is deceased with a history of prostate cancer. His medications include Cetirizine HCl, Compazine, Imodium A-D, Ondansetron HCl, and Pepcid AC. Social History This patient is a 54 year old Caucasian male. He is married with kids and works for a grocery company. Him and his family live in Missouri and have lived there all their life. He is a tobacco user of 25 years 1 pack a day. No alcohol use.
