2 Assessment Plan Introduction This paper will be about a comprehensive treatment plan as well as interview questions a counselor would ask when interviewing a client. Some of the topics within this paper will include questions to assess client background or history, Physical or visual state of the client which would also include non-verbal actions. An assessment plan will be created to assess the full spectrum of problems that the client is currently facing. The last parts will include the plan for treatment (short, long term goals), who will be involved in the clients treatment, and how the treatment will be evaluated or measured. Client Background or History The following questions are designed to elicit a response from the client in order to assess the client’s history. Topics that are to be covered and discussed with the client are identifying information, employment/education history, family information, substance abuse, and if the client has had any legal problems. Based on the questions below there could be additional questions that can be created in order to obtain more information depending on the client’s responses. The Questions are as follows: 1)Hello, before we start I just need to confirm some basic information. Can you please confirm your name, date of birth, address, and telephone number? 2)Please tell me about your current living situation (whom do you live with)? 3)Can you tell me a little bit about the community or neighborhood you current live in? 4)Do you have family active in your life and which members are most active in your life?
