They mentioned about cost because of additional care around $17 Billion to $29 Billion per year for healthcare nationwide. They mentioned about loss of trust and patients fear related to treatment. They mentioned types of errors and how to prevent by considering patient safety. Mostly error happen because of faulty and unorganized system but people assume this is individual mistake that’s why need to work for proper system by which we can resist individual to do any mistake In conclusion they mentioned related to individual carelessness and under- reporting for medical error which leads to re occurrence.
