NURS 6531 N-18 WEEK 2 MAIN DISCUSSION POST 1 NURS 6531 N-18 Week 2 Main Discussion Post Case Study Three A middle-aged gentleman presents to the office with a 2-day history of left earache. He states the onset was gradual but has been getting worse. He states his left ear has been constantly aching since last night and has noticed diminished hearing in left ear. Today, he has noticed some swelling on the left side of his face. SUBJECTIVE: He denies URI, recent fever or chills PMH: DM II, HTN, Hyperlipidemia Allergies: Amoxicillin (pruritus) Medications: Metformin 1000 mg BID, Lisinopril 20 mg daily, ASA 81 mg Daily, Simvastatin 40 mg daily OBJECTIVE: H&W: 5’8, 160 pounds, BMI: 24.3 Vitals: T: 98.8 P: 88 RR: 18, B/P: 138/76 Face: faint asymmetry, Left periauricular area slightly edematous, + tenderness Left preauricular node Eyes: sclera clear, conjunctiva WNL Ears: LEFT: + tenderness pinna, + edema, erythema; + exudate external auditory Canal. TM not visible RIGHT: - tenderness, edema, erythema or exudate Cardiac: S1, S2 regular; No S3 or S4. No murmur Lungs: CTA without rales, wheezes or rhonchi
