Indications Birth of a child or after a miscarriage. CONSIDERATIONS Nursing Interventions (pre, intra, post) Assess your patient's vital signs, breasts, fundus, bladder, lochia, perineum, legs, and any incisions. administer medications as prescribed. watch for signs of respiratory or cardiac distress. watch for signs of hemorrhaging and signs of infection. report any significant findings Outcomes/Evaluation Mother has no postpartum complications or complications have been managed effectively. Potential Complications Postpartum hemorrhage, thrombophlebitis, infections (including mastitis, endometritis, and urinary tract infections), and postpartum depression. ACTIVE LEARNING TEMPLATES Client Education If saturating one pad in less than an hour, a constant trickle of lochia, or the presence of large (i.e., golf–ball sized) blood clots; or foul smelling lochia report this to the nurse. The presence of hot, red, painful, and/or edematous areas in the lower extremities and an elevated temperature should be reported. Encourage patients to ambulate regularly. Massage of the legs should be avoided due to the possibility of dislodging a clot. Nursing Interventions provide mother with resources that can provide support both financially and emotionally. Have patient ambulate as soon as the doctor prescribes. watch for signs of hemorrhaging.
