CHAPTER 101. What were the causes of the Metrolink accident?My view is that the direct cause of the accident was the failure by Robert Sanchez to call out the two signals while he was operating the train. He did make 4 phone calls and send twenty one messages meaning that he was distracted by his phone. He had in earlier times before been warned about the distractions from his phone but he continued using it in his work duty. This makes him the direct cause of the accident. The indirect cause of the accident was the lack of measures on safety that could have prevented and stopped the accident.2. What could have been done to prevent the accident? Was management deficient? Were regulators deficient? Should either have been doing anything differently?
