Directions: Choose 2 weekdays and 1 weekend day to record all dietary intake (including water and alcohol)-Blank forms below. Note: Attach your 3-day food record (list time of meal/snack, amount, and type of food/beverage consumed. Include any additions such as butter, salad dressings, gravy, etc). This can be handwritten (scanned) or typed. 2) Once you record your dietary intake, enter into a calorie counter ( , myfitnesspal app or equivalent) and list your total calories, 3 day average calories, and average intake of macronutrients (carbohydrates, fats, sugars—along with any additional information such as saturated fat, trans fat, polyunsaturated fat, monounsaturated fat, cholesterol, sugar, fiber) and micronutrients (vitamin A, Vitamin C, Sodium, Iron, and Calcium) output from your 3-day food
