Discussion COLLAPSE Description of the Selected Case Patients with advanced age can be a challenge because of the natural aging process that changes how the body metabolizes medications. When a patient is over the age of 50 they often do not have the same kidney function as when they were younger even if their labs do not show a change in their BUN and creatine (Laureate Education, 2012). The patient being discussed in this paper was 74 years old and had early dementia she was prescribed Ativan 1 mg every 4-6 hours as needed for agitation and anxiety. Her overall health was good and she had only a diagnosis of hypertension and hyperlipidemia. The patient was started on the Ativan after she began to have several episodes of anxiety and agitation during therapy in an inpatient rehab floor at the hospital. She was unable to properly metabolize the Ativan and even with such a small dose she became very unsteady on her feet and unable to participate in therapy because she was an increased fall risk. She had had a dose of the Ativan prior to going to bed and then another before therapy that morning. The patient slept for approximately a day and a half causing her to miss 2 days of therapy that was needed in her recovery after orthopedic surgery.
