Diaper Rash : Manifestations Dry, inflamed and pruritic skin. Begins with erythema in perianal region and can progress to macules and papules. Erosions and crusts. (When documenting always define “red” fire engine red, salmon, cherry red, etc.. Intact vs broken skin; smooth vs raised. In folds, entire diaper area, spotty) Common in infants with thrush- common cause is yeast. (candidas) Medications: A&D ointment, Balmex, Desitin, zinc oxide. (Barrier/Prevention) Do NOT apply to inflamed areas because they retain moisture. Antifungal creams and 1% hydrocortisone for candida. Nystatin, Lotrimin, Triamcinolone 0.1% or fluocinolone 0.025% after application of cool compress. Care Plan Assess: new or continued exposure, parents knowledge skin integrity and infants’ hygiene. Type of diaper used, recent changes in soaps, diapers, wipes, medications, foods or lotions. DX : Acute Pain related to skin inflammation - Expected outcome - reduced inflammation, increased healing, decreased irritability, uninterrupted periods of sleep absence of scratching.
