OPERATING AND SUPPORT HAZARD ANALYSIS Instructions: Use the Risk Matrix on page 20 in the textbook to determine risk assessment codes (RAC) while completing the following: 1. Construct an Operating and Support Hazard Analysis worksheet with a minimum of five (5) hazards. 2. Complete a risk assessment using the same format on page 104 of the textbook. Replacement should be done for all faulty servo valves. Also, new training protocols for pilots to handle usual manoeuvres should be introduced. 2 Landing operation Loss of hydraulic pressure Leakage Brakes and Thrust reversers will fail. 2C Operation error Occasional; Results Critical. Proper maintenance and inspection should conducted especially on the hydraulic lines and the filters. 5 Aircraft operator controls Overheat The system exceeds its maximum allowable operating temperature, it may begin to cause damage to the seals. Result in the loss of the hydraulic system and the loss of function of those components that it powers. Also the seals will be damaged and cause leakage. 2C Operation error Occasional; Results Critical. Revise on training manuals to ensure that the system operates within its design limit. Hazard Condition 1: Component failure Cause – What caused such component failure is that, the servo valve in the Power Control Unit (PCU) got jammed. The Boeing 737s always had an issue with its rudders. When the servo valve in the PCU jams, the rudder would be fully deflected to one side.
