Initial post Health risk assessments (HRAs) provide an opportunity to emphasise health promotion and disease prevention for individuals and populations at large. A key component of health risk assessments is the detailed collection of family health history information. This information is helpful in determining risk both for common chronic conditions and more rare diseases as well. HRAs tools may also enhance patient experiences by increasing patient activation and shared decision making, thereby incorporating patient values into the patient–provider encounter and improving adherence to recommendations (Wu & Orlando, 2015). Establishing a positive patient relationship depends on communication built on courtesy, comfort, connection, and confirmation (Ball, Dains, Flynn, Solomon, & Stewart, 2015). Good communication starts with making the patient feel comfortable, build a trusting relation and establish a connection. Try to ask one question at a time and give the patient the opportunity to answer that question before asking another question.
