INTRODUCING THE PRACTICUM 2 Introducing the Practicum Section One: Introducing the practicum 1. Introduction The health care institution that the practicum is undertaken is Cobble Hill Health Center, a nursing home that community-based in Brooklyn, New York. The health care institution offers various services with the notable ones being Palliative care, Adult day care, respite care, Audiology care, sub -acute care, dementia care, Alzheimer care, cancer care, respiratory care, diabetes care, and rehabilitative care. The inter-professional team involves in the practicum includes the registered nurse, the maintenance employees, the administration staff, the maintenance staff and certified nursing assistant. The population that I serve in the practicum site is the elderly. The demographic feature of the population is that the numbers of the elderly have been increasing recently and thus, cases of chronic illnesses related to old age have been on the rise. 2.0 Discussion with the site supervisor a. Major population needs The major needs of the patients in the health care institution are dealing with chronic illnesses like diabetes, knowledge of staff in relation to the life histories of the residents, developing strategies to deal with communication and behavioral changes, the quality of the relationship between direct care staff with the elderly, their families and other members of staff and understanding the preferences and abilities of the elderly patients and their families. b. Factors creating the needs There are several factors that created the needs that the patients face at the moment, and these can be analyzed by each need. Increased number of elderly patients with chronic illnesses Increased direct medical care needs in dealing with chronic illnesses are attributed to to a number of the elderly caused by the rising quality of life among the population brought about by quality health care and dietary practices (Watters,2011). The increased number of the old generation leads to a proportionate increase among those with chronic illnesses and multi-organ failures, calling for increased health care

Understanding life history, behaviors and characteristics of the community Another important need is to understand the life histories, behaviors and characteristics of the residents basing on their cultures, their chief economic activities, and lifestyles. The African Americans and the Hispanic patients have a different culture, which makes it a major barrier towards attaining quality health care. This is in addition to the fact that most of the staff are from the dominant European American cultures and are not well trained in dealing with people from the minority’s cultures making communication and
