Anticoagulants Drug Generic Name: Warfarin, Heparin, Dalteparin, Enoxaparin, Fondaparinux, Tinzaparin, Protamine Sulfate (injection) Drug Brand Name: Coumadin, Hep- Lock, Fragmin, Lovenox, Arixtra, Innohep, Protamine sulfate (injection) Drug classifications: Anticoagulant/ Antithrombotic agent, Antiheparin agent (Protamine) Drug Indication: Prophylaxis and treatment of: Venous thrombosis, PE, and A. fib with embolization. Management of myocardial infarction: Decreases risk of death, subsequent MI, future thromboembolic events. Prevention of thrombus formation and embolization after prosthetic valve Drug dose range: PO, IV (Adults): 2–5 mg/day for 2–4 days; then adjust daily dose by results of INR. Initiate therapy with lower doses in geriatric or debilitated patients or in Asian patients. Hep. 1000U/HR or 15-18 U/kg/hr.
